MarcoTosatti.com (December 21) published a photo where an electronic device is visible on Francis while he kisses and hugs Fabiola Yanez, the concubine of the Argentinean president.
Tosatti showed the picture to a doctor, who identified the device as an insulin dispenser. Clarin.com speculates that it could also be a pump to inject a painkiller for a patient suffering from chronic pain.
A doctor asked by Gloria.tv declared that the device could be any number of things, "Francis could be wearing a Holter Monitor to check his heart rate over 24 hr period. Or it could be an ambulatory blood pressure monitor, which again is worn for 24 hours. An insulin dispenser is not worn on the waist at the back. It’s usually around the abdomen."
The Director of the Vatican Press Office, Matteo Bruni stated that it was a wireless microphone. Bruni is likely right, but Tosatti doesn't believe him which is unsurprising since Bruni is a notorious storyteller.

My guess is sciatica. One of the drugs used in its treatment is an antidepressant. The painkilling mechanism is not fully understood.
Side effects of tricyclic antidepressants (the most common)may include:
Blurred vision
Drowsiness
Dry mouth
Nausea
Lightheadedness on standing up due to a drop in blood pressure (orthostatic hypotension)

Difficulty thinking clearly

I have sciatica (coming from hours of walking during my Roman years and tennis after it) but have never used prescribed medication for it.
Instead, lots of stretching, rolling around on a tennis ball or harder object, and now and then a TENS unit, given to me by the VA.More
